New Patient Information

Our Centralia office is open from 10 a.m. – 4 p.m., Monday, Tuesday, Wednesday, Friday. Initial appointments may be scheduled by telephone. We will need to clarify information such as address, phone number, billing information, your primary care physician and whether a referral is required.

Physician Referral 

If your insurance requires a referral, we ask that you obtain this prior to your scheduled appointment. Without this referral we will be unable to provide services to you at the time of your appointment.

Follow-up Appointments 

We will try to schedule a follow-up appointment at the end of every visit with us. If you call in for an appointment we may need to refer you to the nurse to determine the type of visit required.

Appointment Confirmation 

We have an automated service that will call and confirm your appointment two days before your visit. If you are unable to keep your appointment, please let us know so that we can use the time for another patient.

Scent-Free Clinic 

We strive to maintain a scent-free environment. Please do not wear perfumes or scented lotions to your appointment.
