New Patient Information

Your First Appointment

If you have been referred to our office for a possible surgery or procedure your first appointment will be a consultation with one of our surgeons.

You can make an appointment by calling (360) 827-8100 between 9 a.m. and 5 p.m. Monday through Thursday, or between 9 a.m. and 3 p.m. on Friday.

Our office is closed on weekends and major holidays. Patient office appointment hours vary

New Patient Paperwork

We encourage you to fill out any paperwork prior to your first appointment:

  • Patient History & Physical: The Patient History Form helps us to collect important information that will guide your consultation with our surgeon.
  • New Patient Paperwork: Completion of the New Patient Paperwork allows us to enroll you as a patient at our clinic.
  • Medication Flow Sheet: Please fill out the medication flow sheet prior to your consultation. Also, please bring any medications you have so that we can ensure we are prescribing the correct medication in conjunction with what you are already taking.

Insurance & Payment for Services

We accept most insurances, including Medicare, Medicaid and Labor & Industries. Co-payments, down payments and payments for non-covered services are expected at the time of service. We understand that surgery can be an unexpected hardship and are willing to work out a reasonable payment plan if you need to carry a balance.

Getting Here

We are located in the Providence Medical Plaza next to Providence Centralia Hospital.

Directions from the North (Tumwater/Rochester): Travel down I-5 S and take exit 81 for Mellen St/WA-507 N. Turn right onto Mellen St. Continue onto Cooks Hill Rd. Our building is located on the left hand side, immediately before you reach the hospital.

Directions from the South (Chehalis/Longview): Travel up I-5 N and take exit 81 for Mellen St/WA-507 N. Turn left onto Mellen St. Continue onto Cooks Hill Rd. Our building is located on the left hand side, immediately before you reach the hospital.
