New Patient Information

Thank you for choosing Providence Medical Group. Our Downtown/South Hill Occupational Medicine Clinic is conveniently located at the intersection of 4th Avenue and Division Street on Spokane's lower South Hill. We’re easy to find and offer plenty of free parking.

Our Spokane occupational medicine doctors and staff will perform an evaluation and assessment of your injury and work with your employer to determine job restrictions / modified duty and return to work. We will notify WA Labor and Industries of your assessment, and any changes in the claim as it progresses.

Your First Visit

When you arrive, there will be some paperwork to complete (bring all your injury-related paperwork with you). A certified medical assistant will take your medical history and prepare you for your consultation with an occupational medicine provider.

No-show Policy

Two consecutive no call/no show appointments could result in discharge from care.

Most Health Plans Accepted 

Providence Medical Group accepts most health plans. We bill most insurance plans directly. Be sure you understand what your insurance plan covers, so you aren’t surprised by unexpected charges. Please bring your insurance card to all appointments.

In the event your injured worker's claim is rejected, your secondary insurance will be billed, or you will be responsible for payment.

Billing, Co-payments

Co-pays are due at the time of service. If you aren’t covered by insurance, full payment is due at the time of service, unless other arrangements have been made.

Accepted Forms of Payment 

We accept personal check, cash or credit card (Visa, MasterCard or Discover).

What to Bring

  • Photo ID and insurance card
  • Co-payment (if required by your insurance plan)
  • Any claim-related paperwork.
  • Your supervisor's name and phone number
  • Full list of medications with dosages (including over-the-counter supplements)

Prescriptions

Prescriptions will be written in the office at the time of your visit. Contact our office if you need authorization for a refill.
