For Patients

From the moment you meet our receptionist we are striving to make sure you feel welcome.

We will try to begin to your evaluation on your scheduled appointment time. Please bring a list of all your questions so we can take the time to address each and every one of them. You will leave with a home exercise program and instructions for how to schedule your follow-up appointments. 

What To Bring

We encourage each person to bring:

  • Picture ID
  • Insurance cards
  • Referral from your primary care physician (if required by your insurance plan)
  • Co-payment (if required by your insurance plan)

What To Wear

Please wear lose fitting clothing that allows you to move and perform the tasks related to your physical therapy.

Your Referral

Our clinic is set up to receive referrals from some local physicians. When we receive their referral via e-mail we will call to set up appointment. We will we give verbal instruction on the telephone and our prescription pads which we provide to many local physicians have maps giving helpful directions on our clinic locations.

Billing Information

It is the patient's responsibility to know what is covered by your insurance concerning number of visits and dollar cap amounts. We have a working knowledge of insurances but each policy is different. Our goal is a smooth and comfortable treatment with a pain free insurance encounter.

Additional Information

We will work to evaluate, treatment and set up a home exercise program all your first day.We try to provide evaluation and care without pain. Some procedures and techniques provide more discomfort than others but as rule we DO NOT try to hurt anyone.
