The Sunflower Program

“The blossom of the sunflower seeks the sun throughout the day, no matter how weak the ray. And, once in full bloom, leaves behind a legacy of new life.”

There’s nothing more difficult than having a child with a life-threatening illness. The child and his/her loved ones need special support and care through this difficult time. The team at Providence Support and Care Services serves infants, children, adolescents and families during this journey with The Sunflower Program. 

Through The Sunflower Program, every child with a potentially life-limiting condition and his/her family receive coordinated care to support their choices and opportunities for optimum quality of life.

The program identifies patients who would benefit from palliative care support, and facilitates family conferences to develop a personalized care plan for use in both hospital and hospice.

We also work with patients and families to determine resources – within the hospital and the region – to provide all elements of care and support. We involve all members of the medical care team in developing the best plan for patient and family.

The Sunflower Program also acts as a resource to outlying agencies as home care is coordinated. We provide follow-up to ensure a plan of care is in place after discharge. We can also initiate the Sacred Heart Children’s Hospital’s bereavement program. 

We are committed to honoring the choices and opportunities of the child and family.
