New Patient Information

We would like to take this opportunity to welcome you to our office.

We are pleased you have selected Providence Medical Group General Surgery. If you have any questions or concerns prior to your visit, please feel free to call us at (509) 897-3790.

Before making an appointment, please check with your insurance plan to make sure that the plan includes our physicians and, if needed, the procedure for which you are scheduled.

We see patients by referral from their primary care physician. If you believe you would benefit from our services but do not yet have a referral, please speak to your doctor.

Directions

Providence Medical Group General and Vascular Surgery is located in the Poplar Medical Complex at 301 W. Poplar Street, right next to Providence St. Mary Medical Center.

From Highway 12: Take the Second Avenue exit. Turn right at the base of the ramp onto Second Avenue. Take Second until you reach Alder, then turn right. Providence St. Mary Medical Center is on Alder between 5th and 7th Avenues.

From Highway 125: Highway 125 becomes 9th Avenue. Turn right on Alder Street. Providence St. Mary Medical Center is on Alder between 5th and 7th Avenues.

If you are arriving by bus, there are bus stops outside both the main hospital and the clinic to make it easy for you to reach us.

Before making an appointment, please check with your insurance plan to make sure that the plan includes our physicians. That will ensure you visit is covered by your insurance. Copayments are expected at the time of your appointment.

If you are a self-pay patient (if you do not have any insurance coverage and are not covered by governmental programs), you will be asked to make a down payment at the time of your appointment. If you are unable to make the down payment or pay for your care for financial reasons, please tell us. You may be eligible for assistance.

Insurance cards are requested at every appointment.

We look forward to meeting you on your initial visit to our offices. It is the goal of our staff to provide you with the highest quality of service.

In order to help us best take care of you, please bring the following to your first visit:

  • Health Insurance Card/Information
  • A list of all medications you are taking
  • A list of your questions and concerns
  • If requested, a copy of your MRI / XRAYS / CT scan reports

Forms

Forms may be mailed to you prior to the appointment with instructions to complete them prior to the appointment and bring them in on the day of the first appointment. If you do not receive any forms in the mail, you will be asked to complete them at your appointment if they are needed for your care.
