Thank you for choosing Providence Medical Group Occupational Health. It is our goal to provide you with excellent service. If you are an employer, we will help you protect the health of your workforce. If you are worker who has been hurt on the job, we will strive to help you recover to the fullest extent possible.
For appointments or if you have any questions as you prepare for your first appointment, please call (509) 897-3050.
If you have a new injury, please come to our office at 380 Chase Avenue, Walla Walla, as a walk-in patient. We will treat you, initiate the workman’s compensation claim, and address your work restrictions.
If you are initially seen by one of the Convenient Care providers, your subsequent follow-up visits will be with Dr. G Robert Waring.
If you have initiated your claim through an emergency room, please call the office phone number and let them know that you need an appointment after being seen in the ER.
If you have initiated your claim through another physician’s office, please call us at (509) 522-5895 and our patient services representative will guide you in the process of getting your previous medical records and the steps needed to get an appointment.
Providence Medical Group Occupational Health is in the heart of Walla Walla and can be reached in 15 minutes from nearly anywhere in the city.

From Highway 12: Take the Second Avenue exit. Turn right at the base of the ramp onto Second Avenue. Take Second until you reach Alder, then turn right. Providence St. Mary Medical Center is on Alder between 5th and 7th Avenues. The clinic is behind the hospital at the corner of Chase and Willow Streets.
From Highway 125: Highway 125 becomes 9th Avenue. Turn right on Alder Street. Providence St. Mary Medical Center is on Alder between 5th and 7th Avenues. The clinic is behind the hospital at the corner of Chase and Willow Streets.
If you are arriving by bus, there are bus stops outside both the main hospital and the clinic to make it easy for you to reach us.
Providence Medical Group Occupational Health has experienced staff to assist the patients and employers with the workers compensation process. They are experienced with the billing and authorization requirements that will help streamline the care of the patient.
Providence Medical Group Occupational Health also has client contracts and can bill an employer directly for any drug screens, physicals, or regulatory services that are provided to the employee on behalf of the employer. Our on-site billing staff are ready to assist you with any client- billing needs.
Providence Medical Group Occupational Health has all the forms required to initiate a new workman’s compensation claim.
- Picture ID
- A list of your prescriptions and any other over-the-counter medications or supplements you are taking.
- Payment for any self-pay client services
- X-ray (actual films or CD) and X-ray reports (i.e. MRI, PET scan, CT, bone scan, diagnostic reports
- Pathology report and slides
- Medical records
- Workman’s compensation claim information
For your first visit, please bring
- Transfer of Care card if claim was initiated by other provider
- Release of Medical Records from previous provider
- Health History Questionnaire (This form is given at initial appointment)
- Any documents that were given to you by your employer or claims manager that needs to be addressed by the physician.
