Pre-Register Centralia

Pre-registering online will prepare us for your arrival and save you time when you check into the hospital.

Surgery Patients

Your own doctor will probably tell you when to check in. You will also receive a call from someone at the hospital one or two days prior to your service to confirm your time, give you any special instructions, verify your information and answer any questions you may have.

If you are coming into the main hospital for service, you may want to ask your driver to drop you off at the front entrance of the hospital before parking in the visitors' parking area located between the hospital and S. Scheuber Road.

When you arrive, an Admitting Representative will help you through the registration process using the information you provided in your online pre-registration. You will need to sign our "Conditions of Admission" form. Conditions of Admission forms for minors must be signed by a parent or legal guardian. Once all your paperwork is completed, you will be escorted to your service area.

Unless you were instructed to bring your co-payment with you on the day of your surgery, please do not bring large sums of money or credit cards, valuable clothing or any jewelry, since the hospital cannot be responsible for them. Also, do not bring electrical items from home--e.g. radios, hairdryers, etc. (If you would like to use a radio, please bring a battery-powered one.) Butane-heated hair curlers/irons are not permitted at Providence Centralia Hospital. Expect that any electrical items you do bring will be evaluated for safety by the hospital Bio-Med Department.

Maternity Admissions

Between 6 pm and 5 am you should enter the hospital through the Emergency Department entrance. All other hours, you should enter through the main hospital entrance.

In the "ADMISSION INFORMATION" section of the online pre-registration form, please list a pediatrician or family practice physician who will see your newborn at the hospital. This should be listed in the box entitled "NEWBORN DR."

To help ensure that your insurance will cover the expense of the "newborn visit," please contact your insurance company to obtain a list of pediatricians or family practice physicians who hold privileges at Providence Centralia Hospital, and are participating providers with your insurance company. If a physician is not selected, one will be appointed to provide care to your newborn upon delivery. If the appointed physician is not part of your insurance company's provider panel, the cost of the newborn visit will be passed directly to you. It is important to note that your choice of a physician to see your newborn upon delivery in no way affects which physician you choose to be your child's pediatrician or family physician once you leave the hospital.

Financial Responsibility

Health insurance companies may require you to contact them before your hospital stay so they can approve your stay in advance. Please read through your insurance card to identify the appropriate telephone number to contact.

Please bring your Medicare card, insurance card or current month medical coupon (if covered by a state Medicaid program) with you on the day of your service. This will help us process your claim quickly and efficiently.
