Student Responsibilities

The following links contain information and forms you should review and sign prior to your start date at Providence. A Pre-Arrival Orientation Checklist for Nursing is provided to track your progress.

 

  • Step 1: Read and Review Guidelines (click on titles to access)
                    • Code of Conduct
                    • Appearance Policy
                    • Cultural Diversity
                    • Safety Presentation
                    • Patient Safety
                    • Infectious Disease Control
                    • Pain Management Presentation

  • Step 2: Print, Review and Sign Required Forms (click on form title to access the form; submit signed forms to your school)
                    • Signed Non-Employee Confidentiality Form
                    • Signed Code of Conduct Acknowledgement Form
                    • Signed Acceptable Use Agreement Form

  • Step 3: Print, Review and Sign Required Forms
                    • Accu-Chek Inform II Training Checklist and Quiz (please print and
                      bring with you to the clinical site)
                    • Glucose Meter Policy (please read to prepare for checklist and quiz
                      above)
                    • Point of Care Presentation (please read this information prior to
                      orientation)

PYXIS Tutorial and Access:

  1. Go to Pyxis Tutorial
  2. Set up your own User Name and ID
  3. Once into the program, from the Training Catalog you need to take the CLP1042 Pyxis MedStation 4000 Nursing Tutorial
  4. When you complete the course – at the bottom of the course description screen is the ‘status’ and your certificate
  5. Print your certificate
  6. Complete the PYXIS – Student User Authorization Form
  7. Give your certificate to your instructor along with the completed User Authorization form

Time Logs

Please use this Education Log to record all time you spend in your clinical experience.

Complete each day of clinical experience.

Submit to your instructor or Nursing Education Office at the end of the quarter.
