Providence In-Home Primary Care to close August 6, 2017

We have appreciated the opportunity to be part of the community of providers supporting our elderly patients in private homes, Adult Family Homes and Assisted Living Facilities. With recent changes to our program, Providence is no longer able to provide our In-Home Primary Care (IHPC) practice that serves home bound elders in Snohomish County. The closure of IHPC is effective August 6, 2017.

Providence remains committed to caring for our seniors and will be trialing small pilots of change reconfiguring in-home services within our primary care medical home model as an extension of our primary care services to the very frail. Any future changes will be thoughtfully planned and implemented, and we will update you as we are able.

Providence Medical Group and Providence Regional Medical Center Everett will continue to provide the highest quality integrated and coordinated care, and will transition your care to a PMG provider if that is your wish. Please let us know either by My Chart message or by calling 360-563-5930 in the next thirty days.
