Transitional Care Services
The Transitional Care Unit at Providence Mother Joseph Care Center is designed for patients coming out of the hospital who need additional care. There is a special focus on rehabilitation for those recovering from surgery or who have a functional loss from illness, disease, or an accident.
Under the management of our medical director, and in collaboration with the patient’s primary care physician, the Transitional Care Unit provides an intense, proactive rehabilitation program. The program is intended to return patients to the highest level of independence they can achieve.
In addition, the Transitional Care Unit specializes in the treatment of complex medical conditions. We offer the latest in medical practices and technology, combined with an exceptional standard of care.
