Welcome to Our Practice

In order for us to have a good working relationship and provide you with the highest level of service we feel it is important for you to know more about us.

Clinic Hours

Our office is open from 8:30 a.m. to 4:30 p.m., Monday through Friday, by appointment only. We are closed on major holidays so our employees can enjoy time with their families.

  • Dr. Velikova and her nurse are out of the office every Tuesday.
  • Dr. Jansen and his nurse are out of the office every Wednesday.
  • Dr. Smitherman and his nurse are out of the office every Thursday.
  • Dr. Geraci and her nurse are out of the office every Friday.
  • Dr. Gupta is accepting new patients and will be practicing starting December 3rd. Please call (360) 923-4600 to request an appointment.

New Patient Forms

To save time, you can print out and complete the following forms and bring them to your first appointment:


We are currently contracted with most major insurance carriers. We ask that you bring your insurance card with you as we will take a photocopy for our billing department. As a courtesy, we will submit claims to other insurances provided we have all the information. Please understand insurance policies are between you and the insurer. Please check with them your insurance to ensure you are covered for our services and any services that you are referred for including but not limited to: x-ray, CT scan, MRI, specialty and lab services.

Fees & Payments

Your co-payment is due in full at the time of service; otherwise, we will need to reschedule your appointment. There are a few exceptions such as annual physicals and injenctions. If you have no insurance coverage, payment is also due at the time of service. If you feel that a payment plan is necessary, please contact the business office at 1-866-747-2455 to make arrangements. We accept cash, check, money orders, Visa, MasterCard, American Express and Discover. Any returned checks are subject to a $25.00 NSF fee.

Missed Appointments/Cancellations

We require 24 hours notice of cancellation if you are unable to keep your appointment and a minimum of 1 hour’s notice prior to your appointment time for same day appointments. After 3 failed appointments without proper notice, you will be dismissed from our practice.

We do our best to stay on schedule so we ask that you allow for possible traffic delays, knowing we reserve the right to reschedule your appointment if you arrive more than 10 minutes late.

Unfortunately, there are no exceptions. As a new patient, if your initial appointment is missed without proper notice, you will not be rescheduled. Once accepted into our practice, you will need to make an initial appointment to establish care.

Driving Directions

Directions from the North (Tacoma/Dupont): From I-5 S take exit 111, WA-510 E, toward Marvin Rd/Yelm. Turn Right onto Marvin Rd NE. Pass through 2 roundabouts and you will see our medical building on the left. Free parking is provided in front.

Directions from the South (Olympia/Tumwater): From I-5 N take exit 111, WA-510 E/Marvin Rd toward Yelm. Keep Left to take the Marvin Road North Ramp. Turn Left onto Marvin Rd NE/WA-510. Continue to follow Marvin Rd NE through 2 roundabouts and you will see your building on the left. Free parking is provided in front.
