The first visit will be in your home (or at the hospital if arranged prior to your discharge). The client care coordinator will need to conduct a safety inspection of your home, review your health status, current medications and determine your eligibility for VA, Medicaid or long-term care insurance policy coverage. Please have all of this information available for the initial visit.
Providence DominiCare requires a two-hour minimum for all services in your home.
Providence DominiCare charges on a sliding fee scale, based on your monthly income. Some clients with VA benefits, Medicaid assistance or long-term care insurance policies are eligible to receive our services at greatly reduced rates, and sometimes at no cost.
