Emergency Department Consistent Care Program at Providence St. Peter Hospital wins National Award for Improving Community Health
Olympia, Wash. -- The American Hospital Association (AHA) announced today that five programs across the country – including the Emergency Department Consistent Care Program at Providence St. Peter Hospital – will receive the AHA NOVA Award. The award recognizes hospitals and health systems for their collaborative efforts toward improving community health.
“This year’s NOVA winners are a testament to the caring and compassionate mission of hospitals to improve their communities,” said AHA President and CEO Rich Umbdenstock. “Through leadership and collaboration with like-minded organizations, they have found truly innovative and inspirational ways to better the lives of so many.”

The Consistent Care Program represents an interdisciplinary approach of providing vulnerable members of our community with hope and resources to assist in improving their health."
The Emergency Department Consistent Care Program is an intervention program that guides treatment decisions for frequent emergency department patients with complex health needs. The majority of these individuals have issues regarding mental health, chemical dependency and/or unmanaged chronic conditions.
To date, Consistent Care has assisted more than 630 patients, expanded to include four other hospitals in a five-county region, dramatically reduced emergency visits, and saved millions of dollars in charges. Launched in 2003, the program is operated in collaboration with CHOICE Regional Health Network – a non-profit community-based collaborative care network of rural and urban hospitals, practitioners, public health clinics, community health centers, behavioral health programs and others.

Other winners of the AHA's 2011 NOVA award are: The Diabetes Collaborative in Chicago; Integrated Community Nursing Program at Parkview Health in Fort Wayne, Ind.; Milwaukee Health Partnership, Milwaukee and Glendale, Wis.; and the Rochester Youth Violence Partnership in Rochester, N.Y.
