Long Term Care Services
Our Long Term Care unit provides skilled nursing in a residential setting. We are dedicated to changing the traditional medical model approach to nursing home care by addressing the needs of our residents through creating a sense of community and home. We accomplish this by working together in “neighborhoods” and providing a model of care that is resident-centered.
We impact the world of long term care by:
- Striving to eliminate the pain of loneliness, helplessness and boredom for our residents.
- Providing spontaneous and varied activities including time with pets living at or visiting Mother Joseph Care Center, intergenerational activities with children, and outings.
- Fostering independence and empowerment for residents by encouraging decision making in their daily lives.
- Recognizing the power of interpersonal relationships among residents, families, volunteers and staff working and living in our neighborhoods.
We have four neighborhoods in our long term care community: Emerald Isle, Sunshine Boulevard, Pacific Cove, and Paradise Isle. Our neighborhoods provide a smooth transition for life in a long term care setting. Each resident is assessed and a Plan of Care that fits his or her past and present life-style, medical needs, and social desires is put into place upon admission.
