Providence offers exceptional skilled care for patients of all ages, with all medical diagnoses, within the comfort of their home. Home health patients are those who find it taxing to leave their home. They may have a new diagnosis or illness; be coping with a chronic condition, such as heart failure or diabetes; or recovering from surgery, such as a total hip or knee replacement, or hospitalization.

Home health is for patients who need:

  • Care and teaching related to a newly diagnosed illness, such as diabetes, heart or lung disease, or cancer
  • Care for a worsening illness or condition
  • Follow-up care or rehabilitation from surgery, stroke, illness or injury
  • Education about new treatments or medications (including pharmacy consulting), and monitoring their progress
  • Education for themselves and their family/caregivers how to provide care or how to operate and maintain in-home medical equipment
  • Health care and emotional support during a terminal illness
  • Chaplain services
